Eligibility

Any resident of the United States or American citizen living abroad who is unable to read or use standard print materials as a result of a temporary or permanent visual, physical, or neurological (e.g. dyslexia, etc.) limitation is eligible to receive service.

Certification

Certification of disability must be completed for all applicants. Eligibility must be certified by one of the following: doctor of medicine, doctor of osteopathy, ophthalmologist, optometrist, psychologist, registered nurse, therapist, and professional staff of hospitals, institutions, and public or welfare agencies (such as an educator, a social worker, case worker, counselor, rehabilitation teacher, certified reading specialist, school psychologist, superintendent, or librarian).

How to Apply

Please download the appropriate application to register for service. The applications are portable document format (PDF) files, so Adobe Reader software or other PDF readers is required to view the documents.

Individual Application

Institutional Application – assisted living centers, hospitals, nursing homes, schools, etc.

Public and private schools with general enrollment (eligible and non-eligible students) will need to provide a list of active eligible students (PDF) for the current school year who will be using library materials and equipment issued to the school.

An application for service and information about OLBPD can also be requested by contacting OLBPD.

At this time, completed applications for service can be submitted electronically via fax or emailed as an attachment.
